Clearly, accidents should be investigated so as to understand the underlying problems if we are to prevent a future undesired event.

An unplanned or undesired event that could have resulted in harm to people, damage to equipment or business losses. In simple terms “Wow, that was close!” Worth noting that this could also be called a “near hit!”
Or, to put it another way – a free lesson that something is wrong - “We got away with it today.” We might not get away with it tomorrow!
